1. CAPTOPRIL-DENK

Captopril 12.5mg/25mg/50mg/100mg
ACE Inhibitors 

Indications:

Treatment of heart failure with reduced ejection fraction (HFrEF).
It can also be used to treat hypertension 

Contraindications:
Hypersensitivity to ACEI
Anuria
Angioedema induced ACEI
Contraindicated in pregnant women and breastfeeding

Dosage/How to use:

A.  ADULTS:

Acute Hypertension
12.5-25 mg PO; may repeat PRN

Hypertension (Alone or with Thiazide)
Initial: 25 mg PO q8-12hr, increase gradually based on response (may start lower in some patients)
Maintenance: 25-150 mg PO q8-12hr
450 mg/day maximum

Congestive Heart Failure (With Diuretics and Digitalis)

Initial: 6.25-12.5 mg PO q8hr in conjunction with cardiac glycoside and diuretic therapy

Target therapy: 50 mg q8hr

450 mg/day maximum

 Left Ventricular Dysfunction After Myocardial Infarction

6.25 mg PO initially followed by 12.5 mg q8hr
Increase to 25 mg PO q8hr over next few days; THEN
Target dose: 50 mg PO q8hr

Diabetic Nephropathy
25 mg PO q8hr

B.  PEDIATRICS:

Hypertension (Off-label/unproved)

Neonates: 0.05-0.1 mg/kg/dose q8-24hr, titrate dose up to 0.5 mg/kg/dose q6-24hr  

Infants: 0.15-0.3 mg/kg/dose; titrate dose upward to maximum 6 mg/kg/day in 1-4 divided doses; 2.5-6 mg/kg/day usually required

Children: 0.3-0.5 mg/kg/dose; titrate to maximum 6 mg/kg/day divided q6-12hr

Older children: 6.25-12.5 mg/dose q12-24hr; titrate to no more than 6 mg/kg/day divided q6-12hr

Adolescents: 12.5-25 mg/dose q8-12hr; may increase by 25 mg/dose q1-2Weeks to a maximum of 450 mg/day.


2.  ENA-DENK

Enalapril 10mg/20mg
ACE Inhibitors 

Indications:

Treatment of heart failure with reduced ejection fraction (HFrEF).
It can also be used to treat hypertension 

Contraindications:
Hypersensitivity to ACEI
Anuria
Angioedema induced ACEI
Contraindicated in pregnant women and breastfeeding

Dosage/How to use:

A. ADULTS

Hypertension:

10-40mg/day PO, divided into q12hr.

Left ventricular dysfunction

20mg/day po

Congestive heart failure

10-40mg/day PO, divided q12hr

B. PEDIATRICS

Hypertension:

1 month-16 years:0.08mg/kg/day PO, divided q12hr or IV 0.01-0.02mg/kg/kg/day

Hypertensive crisis:

Use enalapril IV: 0.05-0.1mg/kg/day iv direct

Renal impairment:

GFR<30ml/mi/1.73m2:Not recommended



3. ZESTORETIC

Lisinopril 20mg/Hydrochlorothiazide 12.5mg

Indications:

Treatment of heart failure with reduced ejection fraction (HFrEF).
It can also be used to treat hypertension 

Contraindications:
Hypersensitivity to ACEI
Anuria
Angioedema induced ACEI
Contraindicated in pregnant women and breastfeeding

Dosage/How to use:

 A.ADULTS:

Hypertension:

10-80mg lisinopril/6.25-50mg hydrochlorothiazide qDay

Heart failure:

10-80mg lisinopril/6.25-50mg hydrochlorothiazide qDay

B. PEDIATRICS:

Safety and efficacity not established

4. AMLOZAAR-H

Losartan potassium 50mg + Amlodipine 5mg+Hydrochlorothiazide 12.5mg

Indications:

Treatment of moderate to severe hypertension 

Treatment of heart failure

Contraindications:

Breastfeeding and Pregnancy in second and third trimesters

Dosage/How to use:

Hypertension with left ventricular hypertrophy:

Adult: The initial dose is 1 tablet(50mg/12.5mg) daily

Not recommended for children below 6 years


5. CARDIURINE

Captopril 50mg/hydrochrolothiazide 25mg
ACE Inhibitors 

Indications:

Treatment of heart failure with reduced ejection fraction (HFrEF).
It can also be used to treat hypertension 

Contraindications:
Hypersensitivity to ACEI
Anuria
Angioedema induced ACEI
Contraindicated in pregnant women and breastfeeding

6. ZIAK

Bisoprolol Fumarate 10mg+ Hydrochlorothiazide 6.25mg

A beta blocker

Indications:

Treatment of moderate to severe hypertension 

Treatment of heart failure

Contraindications:

Cardiogenic shock, Overt cardiac failure, Second or Third AV block,  Marked sinus bradycardia

Not contraindicated in pregnancy and breastfeeding, but you should consult your Doctor first

Dosage/How to use:

Hypertension:

Adult: The initial dose is 1 tablet daily

Pediatrics: Saftey not established

Heart Failure:

Adult: The initial dose is 1 tablet daily

Pediatrics: Saftey not established


7. TORSINEX

Torasemide 5mg/10mg/20mg

Diuretics

Indications:

Treatment of moderate to severe hypertension 

Treatment of  fluid overload

Contraindications:

Anuria, Hepatic comma, hypersensitivity to torsemide or povidone

Dosage/How to use:

Edema:

Adults:10-20mg PO daily

Pediatrics: Safety not established

Hypertension:

Adults: 2.5-5mg PO daily

Pediatrics: Safety not established

Liver Cirrhosis:

Adults:5-10mg PO daily

Pediatrics: Safety not established

Adult: The initial dose is 1 tablet daily

Pediatrics: Saftey not established


8. LASIX

Furosemide

Diuretics

Indications:

Treatment of moderate to severe hypertension 

Treatment of  fluid overload

Side effects:

Hypokalemia, Hyperuricemia

Contraindications:

Anuria, Hepatic comma, hypersensitivity to furosemide

Not contraindicated in  pregnant and  breastfeeding women

Dosage/How to use:

Edema:

Edema associated with heart failure, liver failure, or renal failure 

Adults:20-80mg PO daily or IV/IM 20-40 mg daily

Pediatrics: 1-2mg/kg IV/IM/PO twice daily.

Neonate < 28 days: 0.5-1mg/kg IV/IM q8-24hrs

Hypertension:

Adults: 20-80mg PO twice a day

Pediatrics > 1 year: 0.5-2mg/kg PO q12-24 hrs

Children < 1 year: safety and efficacy not established

Acute pulmonary edema/hypertensive crisis/increased intracranial pressure:

Adults:0.5-1mg/kg(or 40mg) IV over 1-2min.

Pediatrics: Safety not established


9. ESIDREX

Hydrochlorothiazide

Diuretics

Indications:

Treatment of moderate to severe hypertension 

Treatment of  fluid overload

Contraindications:

Anuria,  hypersensitivity to hydrochlorothiazide

Not contraindicated in  pregnant and  breastfeeding women

Dosage/How to use:

Edema:

Edema associated with heart failure, liver failure, or renal failure 

Adults:25-100mg PO daily or twice a day

Pediatrics: 1-2mg/kg IV/IM/PO twice daily.

Neonate < 28 days: 0.5-1mg/kg IV/IM q8-24hrs

Hypertension:

Adults: 12.5-50mg PO daily.

Children < 6 months: 1-3mg/kg/day PO q12hrs

Children 6months-12 years: 1-2mg/kg/day PO q12hrs or once a day

Children 2-12 years: 1-3mg/kg/day

Hypertension in heart failure:

Adults:25 mg po once or twice a day
